    • Insurance benefits verification, copay disclosure, and digital consent captured orally or in writing
    • Weekly enrollment reporting so clinical leadership always knows where the patient enrollment and participation stands
  3. Phase 3

    Connected Devices, Delivered

    Patients receive ready-to-use, cellular-connected monitoring devices at home — no app downloads, no Wi-Fi setup, and no calls to your front desk for tech support.

    • FDA-cleared devices selected from 11 qualified manufacturers that offer over 100 devices to address each patients chronic condition
    • Pre-paired and pre-activated equipment shipped directly to the patient's residence or LTC facility
    • Welcome call to confirm the patient's first transmitted reading and walk patients through program expectations
    • HealthIQ owns ongoing supply, replacements, and warranty escalations
  4. Phase 4

    Active Clinical Oversight

    Our U.S.-based clinical staff acts as a 24/7 safety net — reviewing daily readings, calling patients, and escalating concerns directly to the treating provider.

    • Personalized monthly engagement that builds patient trust and drives medication adherence
    • Provider-defined clinical thresholds to notify treating providers of escalating patient vital signs
    • Clinical summaries that condense weeks of patient clinical data into existing practices notes
    • Documentation from the patients vital measurements along with feedback into the patients EMR to keep charts current
  5. Phase 5

    Reimbursement & Reporting

    We deliver clean, audit-ready time logs and CPT-mapped billing files each month so your revenue cycle team can submit claims with confidence.

    • Monthly billing packages aligned to RPM, CCM, PCM, BHI, RTM, and APCM codes
    • Direct EHR write-back or flat-file export to match your billing workflow
    • Audit support, denial review, and payer-specific guidance included
    • Optional revenue cycle managment can be provided to the practice at their request
  6. Phase 6
