Connected Devices, Delivered
Patients receive ready-to-use, cellular-connected monitoring devices at home — no app downloads, no Wi-Fi setup, and no calls to your front desk for tech support.
- FDA-cleared devices selected from 11 qualified manufacturers that offer over 100 devices to address each patients chronic condition
- Pre-paired and pre-activated equipment shipped directly to the patient's residence or LTC facility
- Welcome call to confirm the patient's first transmitted reading and walk patients through program expectations
- HealthIQ owns ongoing supply, replacements, and warranty escalations
- Phase 4
Active Clinical Oversight
Our U.S.-based clinical staff acts as a 24/7 safety net — reviewing daily readings, calling patients, and escalating concerns directly to the treating provider.

- Provider-defined clinical thresholds to notify treating providers of escalating patient vital signs
